The focus is on enabling, empowering and supporting them to remain independent, active and engaged members of their communities, both receiving and providing the support and activities they identify that they need and which are their chosen priorities.
We are looking for someone to support the day-to-day delivery of AUKBD’s Social Connections work with a specific focus on the Enabling Connections Programme, providing person-centred support to older people experiencing loneliness and social isolation through a structured, time-limited, goals-oriented befriending model. We would love to hear from people who are passionate about supporting older people to live their best possible life.
To support the day-to-day delivery of AUKBD’s Social Connections work with a specific focus on the Enabling Connections Programme, providing person-centred support to older people experiencing loneliness and social isolation through a structured, time-limited, goals-oriented befriending model.
The key responsibilities for the role are;
- Coordinate referrals and undertake initial conversations and assessments with older people referred into the programme.
- Support individuals to identify personalised goals and develop friendship plans.
- Coordinate appropriate pairing between volunteers and older people based on shared interests, preferences, and needs.
- Monitor participant progress and undertake regular reviews.
- Support planned endings and transitions into sustainable community connections.
- Ensure all relevant data is captured on CRM system

Strong administration and communication skills are required for the role, together with a positive attitude and willingness to learn.
Please note that this role is subject to a DBS check, which will be arranged and paid for through Age UK Bradford District.
Salary £24,454.04 FTE per annum, actual £13,879.32 per annum
(based on 21 hours per week)
5 weeks holiday plus bank holidays pro rata
Training will take place at our Wrose Community Hubs and the successful candidate will need to be flexible with working days through induction period
